Cancer Treatment for HER2-positive Breast Cancer with Liver Metastases

A Malaysian, female, in her 40s was first diagnosed with left breast cancer with liver metastases in December 2021. Over the next two years, she underwent multiple lines of treatment including chemotherapy, targeted therapy, low-dose radiation, and immunotherapy. By October 2023, it was heart relieving when her scans showed resolution of nodules, and her condition appeared stable.


However, very soon in less than a year, mid-2024, she experienced a recurrence. Her tumour marker began to rise, and subsequent imaging in July confirmed new liver metastases and a mildly FDG-avid axillary lymph node. A liver biopsy and PET/CT scan revealed progressive disease, with multiple new lesions in the liver. The disease was no longer reactive to her previous drugs.


Following this setback, she began ECCT immediately as her Cancer Treatment for HER2-positive. This was combined with oral chemotherapy and monthly targeted injections. ECCT was applied to cover her tumour areas and whole body covering all tumour to ensure full body being treated systemically and also to prevent any potential spread.

In just over a month, imaging showed marked partial resolution of the liver metastases, with only two small nodules remaining. By March 2025, a repeat PET/CT scan revealed complete resolution of all previously FDG-avid liver lesions and axillary lymph nodes. There was no evidence of active disease in the breast, liver, or any distant organs.


Throughout her ECCT treatment, she maintained a good quality of life. She remained physically active, able to sleep and eat well, with only mild fatigue and reduced appetite after treatment sessions.


As of her latest follow-up, she continues her ECCT and monthly targeted therapy with no signs of active metastatic disease. Her case highlights how ECCT, when used as part of an integrative treatment approach, may contribute significantly to tackle the disease and continue as preventive measure from recurrence.
